Job Description:
A company is looking for a Bilingual Cantonese and English
Telephonic Care Navigator.
Key Responsibilities:
Conducts telephonic outreach to members and educates them on care
gaps
Coordinates care with providers and assists members with social
determinants of health
Maintains schedule adherence and provides excellent customer
service to members and providers
Required Qualifications:
High School Diploma / GED or equivalent work experience
Must be 18 years of age or older
1+ years of Call Center and/or Telephonic Customer Service
Experience
Bilingual in Cantonese and English
Ability to work from home with access to high-speed broadband
